You must complete and sign a medical statement form before you can SCUBA Dive.

which you must print, complete, sign and return to us with your Booking Form and course fee.
Print the form and write in the words "YES" or "NO" against each question in the medical questionnaire. You must answer each question HONESTLY !!
Then sign and date each page (if you are under 18 your parent or guardian must also check and countersign your forms).
If you have honestly answered "NO" to each question, thats all you have to do - just return the signed pages to us with your booking form.

you had to answer "YES" to any question on your Medical Statement form, read on:
Dont worry, it doesn't necessarily mean that you wont be able to SCUBA Dive, but we will need to ask you some more questions first: please contact us by email, text or phone.

Take the form to your GP and/or specialist treating your condition for his/her opinion as to whether your condition is suitable for diving. Make it clear to your GP that you are asking his/her advice about engaging in a lifestyle activity with respect to the management of your condition, and NOT asking for a Diving Medical (for which there would be a charge).
Your GP, or specialist treating your condition, is normally in the best position to know your individual circumstances and medical history, but may not know about SCUBA Diving. If your GP is unable or unwilling to sign your form, or his/her medical opinion is that you are not fit to SCUBA Dive, you have the option for a specialist Diving Physician to examine you and give a specialist opinion (for which there would be a charge).
